An annual permit fee of one thousand dollars ($1,000) shall be levied upon each such facility affected by this chapter and shall be paid on or before January 1 of each year. If the fee is not received by the Department of Health on or before it is due, a penalty of twenty-five percent (25%) shall be imposed and paid.
