After the issuance of the permit, the Department of Health shall make periodic, unannounced inspections of the premises whenever it deems necessary and in no event less than twice per year. Within thirty (30) days before the one year anniversary of the issuance of the initial permit, the “pain clinic”, “pain management clinic” or “pain management center” shall submit an application to renew its permit to operate. The same provisions herein shall apply. Thereafter, the renewal requirement shall occur every three years.
