In addition to the affidavit required above (Section 769.03) a separate affidavit shall be submitted by the President and/or Chief Operating Officer of the properly accredited hospital facility or accredited college, university or institute of higher learning attesting that the same “pain clinic”, “pain management clinic” or “pain management center” is a component part of the services provided by said hospital facility or institute of higher learning and is fully integrated operationally and administratively therein.
