769.03 AFFIDAVIT OF ACCREDITATION.
   Any applicant for any permit pertaining to the establishment of a medical office or clinic or medical services facility substantially dedicated to the treatment of pain by the prescription and/or dispensing of controlled substances, shall in addition to the above required application, be required to submit to the Portsmouth Department of Health an affidavit signed by the owner and/or authorized representative of the “pain clinic”, “pain management clinic” or “pain management center” that the operation of such a service, office or clinic is a component part of the services provided by an accredited hospital facility or an accredited college, university, or institute of higher learning and fully integrated operationally and administratively therein. No application shall be accepted, processed or approved without such a written affidavit.
