(a) Every applicant for a license as a driver of a taxicab shall make application to the City Manager on blank forms to be supplied by the City Manager, which forms shall direct applicants to furnish such information as to themselves as shall be required by the application of the City Manager and such application shall be sworn to by the applicant before filing.
In addition to the written application prepared by the City Manager, the applicant must provide:
(1) Valid Ohio Driver’s License;
(2) Proof of lawful presence and proof of identity;
(3) Ohio Bureau of Motor Vehicles driver’s abstract;
(4) Letter of good standing from the City Tax Division; and
(5) PCI background check;
(6) Proof of insurance.
