One copy of such form shall be delivered to the applicant or licensee and shall be the only acceptable evidence of the issuance thereof in the event of the loss or misplacement of the remaining copy or copies. One of such remaining copies shall be filed in the office of the Auditor, except licenses, permits or approvals issued by the Police Department or Port Clinton Waterworks, in which cases one such copy shall be kept in the issuing office.
