Any person, firm or corporation, desiring a license for the use or operation of such device, shall file an application therefor with the City Clerk, upon a form provided by him, setting forth the name and address of the applicant, the name of the owner of the device, the date upon which it is intended to be used, and such other information as may be prescribed.
