860.04   LICENSE APPLICATION AND APPLICATION FEE.
   Applicants for a license under this chapter must file with the Fiscal Officer a sworn application in writing, in duplicate, on a form to be furnished by the Fiscal Officer, which application contains the following information:
   (a)   The name and a description of the applicant;
   (b)   The permanent home address and full local address of the applicant;
   (c)   A brief description of the nature of the business and the goods to be sold;
   (d)   If employed, the name and address of the employer, together with a description of the applicant's relationship to his or her employer;
   (e)   The length of time for which the right to do business is requested;
   (f)   The days of the week and hours during each day that the applicant requests to perform the temporary vending;
   (g)   If the vending is to take place from a fixed location, evidence of the property owner's permission to the applicant allowing such vending;
   (h)   If goods, wares, merchandise or produce is being sold or offered for sale, the place of manufacture or production of such goods, wares, merchandise or produce and the proposed method of delivery to the purchaser, and a copy of all required licenses permitting the vending of all such goods, wares, merchandise or produce;
   (i)   If services are being offered for purchase or hire, the identity of each person, company or other entity who or which will actually provide such services, along with a copy of all licenses permitting each such person, company or other entity to offer and perform such services;
   (j)   A recent photograph of the applicant and of every agent who will participate in the temporary vending;
   (k)   A statement as to whether or not the applicant or any of the vending agents has been convicted of any crime or violation of any ordinance, the nature, location and time of the offense and the punishment or penalty assessed therefor;
   (l)   A fee of twenty-five dollars ($25.00) for processing the application, including an investigation of the facts set forth therein.  However, the application fee shall be waived for applicants who, upon evidence sufficient to satisfy the Mayor, demonstrate that they are vending for not-for-profit organizations having a charitable, civic, cultural, educational or similar community purpose.
