Application for concrete work involving curb, gutter, sidewalk or driveway shall be made by the owner or his agent on forms provided by the building official. Such application shall set forth the property and location upon which the proposed work is to be done, describing the same by lot and block number, or other description by which the same may be readily located and identified, the general dimensions of the driveway, curb, gutter or sidewalk to be constructed, the name of the owner, the name of the person who is to furnish the material, and the name of the person who is to do the work.
