The application for license shall contain the following information:
A. Name, description, social security number, driver's license number and date and place of birth of the applicant.
B. Permanent home address, telephone number, and local address and telephone number for the applicant.
C. A brief description of the nature of the business and goods to be sold or services to be performed.
D. If employed by another, the address and name of the employer.
E. The place of manufacture or production of goods to be offered for sale, the present location of such goods and the proposed method of delivery.
F. A Polaroid picture of each applicant seeking a license to sell door to door to residences, shall be taken by the city clerk at the time of application and shall show the head and shoulders of the seller in a clear and distinguishing manner.
G. The names of two (2) reliable persons residing in the state, as references to the good character and business responsibility of the applicant or, in lieu of said references, the means of obtaining evidence as to the applicant's character and business responsibilities.
H. A statement as to any convictions of crimes, misdemeanors, or violations of municipal ordinances, the date, the nature of the offense and the penalty assessed therefor for the applicant and each employee going door to door.
I. A statement that a license, if granted, will not be used or represented as an endorsement by the city for solicitations thereunder. (Ord. 322, 9-14-1995)
J. The application shall be sworn to by each applicant and shall be accompanied by an application fee for each person to be covered by such license. (Ord. 378, 5-11-2006)
K. As to telephone solicitors and open air markets, the foregoing information shall be required only from the person, group or agency organizing and managing such business operation. (Ord. 369, 5-19-2005)
L. The applicant's Idaho state sales tax certificate or temporary sales tax permit.
M. If a vehicle is to be used, a description of the vehicle including the license number.
N. When food or other products for human consumption are to be sold, a certification by the applicable health agency shall be required prior to issuance of the registration. (Ord. 356, 8-21-2003)
O. If the license is for the operation of an open air market, a copy of the market regulations shall be attached to the application.
P. In the case of an open air market, the applicant shall maintain a list of each and every seller and coseller offering goods for sale at the market, and such other information as may be, from time to time, required by the city. (Ord. 369, 5-19-2005; amd. Ord. 378, 5-11-2006)
