§ 150.63 APPLICATION FOR PERMIT.
   (A)   To obtain a building permit from the Building Inspector, the applicant must make application to the Building Inspector in duplicate on blanks to be furnished by the city.
   (B)   He or she shall include with said application the following information:
      (1)   A sketch of the outside wall of the ground floor of the improvement to be built, drawn to scale and showing its measurements, the distance from the lot lines on which the improvement is to be built, the street and alley, lines and the nearest buildings or structures located on the same lot and adjourning lots;
      (2)   The name of the owner of the real property to be improved;
      (3)   The name of the contractor, if any, doing the work;
      (4)   The legal description of the property;
      (5)   The estimated cost of said improvement;
      (6)   The estimated completion date for said improvements;
      (7)   A general description of the improvement and what type of building materials of which it will be constructed;
      (8)   The use for which the improvement is to be made;
      (9)   A plan or description of the manner of construction of the proposed plan;
      (10)   Plans for the installation of plumbing, electrical wiring and devices necessary for the use of gas and gas implements; and
      (11)   Such other information the Building Inspector may request so that he or she may determine if said improvement complies with the ordinances of the city and the laws of the state.
