An application for a permit for the Bell Tower monument shall be filed with the chief of police of the city and shall state the following:
(A) The name and address of the applicant;
(B) The name and address of the person, group or organization conducting the activity;
(C) The nature of the proposed use or activity;
(D) The date and hours for which the permit is desired; and
(E) An estimate of attendance. (Ord. 1519, 2-28-1995)
