§ 113.15 APPLICATION.
   (A)   No person, except a Class A solicitor, shall engage in residential soliciting without first making written application and obtaining a Certificate of Registration as provided by this subchapter.
   (B)   Application for a certificate required by this subchapter shall be made upon a form provided by the Chief of Police and filed with the Chief. The applicant shall state truthfully and in full the information requested on the application, as follows:
      (1)   The name and address of the applicant's present place of residence, the length of residence at that address, the applicant's business address, if other than the residence address, and the applicant's Social Security number;
      (2)   The address of the applicant’s place of residence during the past three years, if other than his or her present address;
      (3)   The applicant's age and marital status; if married, the name of the applicant's spouse;
      (4)   A physical description of the applicant;
      (5)   The name and address of the person, firm, corporation or association by whom the applicant is employed or represents, and the length of time of such employment or representation;
      (6)   The name and address of the applicant’s employer during the past three years, if other than the present employer;
      (7)   A description, sufficient for identification, of the subject matter of the soliciting the applicant will engage in;
      (8)   The period of time for which the certificate is applied for;
      (9)   The date, or approximate date, of the latest previous application for a certificate under this subchapter, if any;
      (10)   Whether a certificate issued to the applicant under this subchapter has ever been revoked;
      (11)   Whether the applicant has ever been convicted of a violation of any of the provisions of this chapter or the ordinance of any other Illinois municipality regulating soliciting;
      (12)   Whether the applicant has ever been convicted of the commission of a felony under the laws of this or any other state or a federal law of the United States; and
      (13)   Additional information the Chief may deem necessary to process the application.
   (C)   All statements made by the applicant upon the application or in connection therewith shall be under oath.
