§ 52.08 APPLICATION FOR SERVICE.
   Application shall be made to the town for any utility service that may be furnished by the town and may be required in writing. The application shall state the name and address of the applicant, the type of utility service desired, the purposes for which the application is made and any other information as the town may request.
