If the charges for such abatement, including costs, expenses and attorney's fees, remain unpaid, a notice of lien shall be filed in the office of the Perry County Clerk, within one year after the cost and expense for the abatement was incurred. The notice of lien shall consist of a sworn statement setting out:
(A) A description of the real estate sufficient for identification thereof;
(B) The amount of money representing the cost and expense incurred or payable for the service; and
(C) The date or dates when said cost and expense was incurred by the city.
