§ 921.04 SCHEDULE OF RATES.
   (a)   Schedule I.
      (1)   Applicability. Applicable within the entire territory served.
      (2)   Availability. Available for residential, domestic, commercial and industrial services; to colleges; and to hospitals.
      (3)   Rates (customers with metered water supply).

      (4)   Minimum bill. Each customer shall pay a minimum charge of $20.90 per month (equivalent to 2,000 gallons of water usage).

      (5)   Flat rate charge. $47.03 per month; customers with non-metered water supply; equivalent to 4,500 gallons of water usage.
      (6)   Delayed payment penalty. The above schedule is net. On all current usage billings not paid in full when due, 10% will be added to the net current amount unpaid. This delayed payment penalty is not interest and is only to be collected once for each bill where it is appropriate.
      (7)   Resale rates.

      (8)   Customer deposits. As provided for by West Virginia Code and Public Service Commission Rules and Regulations.
      (9)   Water service connection charge. The following charges are to be made whenever the utility installs a new tap to serve an applicant.
         A.   A tap fee of $100 will be charged to customers applying for service before construction is completed adjacent to the customer’s premises in connection with a certificate proceeding before the Commission. This pre-construction tap fee will be invalid after the completion of construction adjacent to the applicant’s premises that is associated with a certificate proceeding.
         B.   A tap fee of $350, or the actual cost of the connection (solely determined by the city), whichever is greater, will be charged to customers who apply for service outside of a certificate proceeding before the Commission for each new tap to the system.
      (10)   Disconnect charge/reconnection charge/administrative fee.
         A.   Water service will not be restored until all past due water bills have been paid in full and all accrued penalties plus a disconnection charge of $30 have been paid.
         B.   There shall be a $30 reconnection charge paid prior to restoration of water service which had been previously disconnected for any reason.
         C.   In the event that city staff or agents collect money at the customer’s residence or otherwise give the customer the opportunity to pay the bill at the office in order to stop disconnection, an administrative fee of $30 shall be paid in addition to other charges to prevent disconnection.
      (11)   Returned check charge. A service charge of $25 will be imposed upon any customer whose check for payment of charges is returned by the bank due for any reason.
      (12)   Incremental costs. An amount not to exceed $5 per 1,000 gallons is to be used when the bill reflects unusual consumption which can be attributed to eligible leakage on the customer’s side of the meter. This rate shall be applied to all such unusual consumption above the customer’s historical average usage. The city shall establish a nondiscriminatory policy regarding this provision for leak adjustments.
      (13)   EFT, credit card and drop box payments. A service charge will be imposed on EFT, credit card or drop box payments. The amount shall be equal to the actual charges to the city from the financial institution for processing payment.
      (14)   Multiple occupancy. On apartment buildings or other multiple occupancy buildings, each family or business unit shall be required to pay not less than the minimum charge herein established for a five-eighths inch meter, the minimum bill for the size of meter installed, or the actual charge for water used, whichever is greater. Motels and hotels shall pay according to the size of the meter installed.
   (b)   Schedule II fire protection schedule.
      (1)   Applicability. Applicable in all areas served by the city.
      (2)   Availability of service. Available for public and private fire protection service.
      (3)   Private rates. Where connections, hydrants, sprinklers, and the like, on property maintained by customer.

      (4)   Periodic billing. The schedule of rates and charges may be applied on a monthly, bi-monthly, quarterly, semi-annual or annual basis, provided no lower charges apply, at customer’s request.
      (5)   Public rates. Six thousand eight hundred dollars per annum, payable in 12 monthly installments. The city is to be entitled to as many fire hydrants as it may deem necessary; the said city to furnish said hydrants and to pay for installation thereof, and the water department to maintain said hydrants.
