The application for a peddler's or canvasser's license shall be made to the Office of the Clerk-Treasurer, and shall set forth:
(A) The name under which the business is to be conducted.
(B) The name, address and telephone number of the applicant.
(C) The driver's license number and the license plate number of the vehicle to be used, if any.
(D) The location(s) where the applicant intends to do business.
(E) The type of goods, wares or merchandise the applicant intends to sell.
(F) The length of time for which the applicant intends to do business.
(G) Proof that the business entity that is sponsoring the applicant exists, including the name, address and telephone number of the sponsoring agency or business and, in the case of out of state businesses or organizations, the name, address and phone number of the Indiana registered agent.
(H) The result of a limited adult criminal history check by the State of Indiana. Applications shall be provided by the City Clerk-Treasurer and shall be the form prescribed by the State Board of Accounts.
