(a) A person desiring to engage in the construction, reconstruction or making of walks, driveways or curb openings in the City shall apply to the Director of Public Service-Safety for a license to engage in such activity, upon forms furnished therefor. Such application shall set forth the following information:
(1) In the case of an individual, his full name and address;
(2) In the case of a partnership, the full name of the partnership, evidence of the legal organization of the partnership in accordance with the laws of the State, the business address and the full names and addresses of the persons in the partnership; or
(3) In the case of a corporation, the name of the corporation, the state of incorporation, the name, title and signature of the official or officials authorized to sign the application, and such further information as may be required by the Director.
(b) The applicant shall further furnish the Director with information as to his ability to do and perform all work required in the construction or reconstruction of walks, driveways or curb openings. (Ord. 83-74. Passed 2-11-75.)
