(a) Each application for a permit hereunder shall contain the name, social security number/tax identification number, address and telephone number of the person, firm, partnership, corporation, company, association, or other legal entity applying for such permit, whether the applicant is a member of a firm, partnership, corporation, company, association or other legal entity or any combination thereof, and evidence showing that such applicant is duly authorized to act for such firm, partnership, corporation, company, association or other legal entity, or any combination thereof, as well as such other information as the Mayor shall prescribe.
(b) No permit shall be issued by the Village authorizing any person, firm, partnership, corporation, company, association, or other legal entity, or any combination thereof, to engage in the activities or work referenced in Sections 1321.01(b) or 1321.09(a) of this Chapter, until evidence is provided to the Mayor that the applicant is registered with the Village.
(c) Said registration with the Village must be completed to the full satisfaction of the Mayor prior to the start of the activities or work referenced in Sections 1321.01(b) or 1321.09(a) of this Chapter. Any person, firm, partnership, corporation, company, association, or other legal entity, or any combination thereof, must maintain a current registration with the Mayor to insure the issuance of any required permits by the Zoning Inspector shall include the applicant's name, address, social security number and/or tax identification number, and principal place of business, and, in the case of a corporate or limited liability company entity, the name, address and tax identification number and the statutory agent or managing member. Upon compliance, a permit shall be issued by the Mayor. Any Contractor acting as a "general contractor," "construction manager" or a similar position of responsibility, shall provide to the Mayor verification that any and all other Contractors (acting in the capacity of a subcontractor) working, operating or conducting business and working at their direction or request have complied with the provisions of this Chapter. If the identity of the subcontractor is unknown at the time of the initial registration of a contractor, such registration shall be permitted, upon compliance with all other provisions herein. Upon determining any subcontractors now known at the time of initial registration, the contractor, as well as any subcontractors, shall be responsible for providing within ten (l0) days the information as required above to the Mayor of the Village. Failure to do so shall result in revocation of the permits previously issued.
(d) Revocation shall also occur in the event of failure to file tax returns, including withholding returns, failure to be current in the obligation to file tax returns, including withholding returns, failure to withhold and/or pay any taxes, interest or penalties owed to the Village, failure to be current in the obligation to pay any taxes, interest or penalties owed to the Village, or for other good cause articulated by the Village of Perry.
(e) Any fees for the permits required herein shall be set annually by Council, and shall bear a reasonable relation to the work and/or forms expense involved in such registration and the issuing of such certificate.
(f) The Mayor may provide written notice ordering any person, firm, partnership, corporation, company, association, or other legal entity, or any combination thereof in noncompliance with any provision of this Chapter, to cease and desist activity or work until that person, firm, partnership, corporation, company, association, or other legal entity complies with the provisions of this Chapter. Until compliance is attained, an injunction and/or restraining order may be granted by a court of competent jurisdiction to prevent further violations of this Chapter under penalty of law as set forth herein.
