The subdivider shall furnish the following:
(a) Final Plat Application. (See Appendix F.)
(b) Final Plat Drawings. The final plat shall be drawn in ink on tracing cloth or mylar (sheet size 24" x 36") and shall be at a scale of not more than on hundred (100) feet to one (1) inch. Where necessary, the plat may be on several sheets accompanied by an index sheet showing the entire subdivision. The final plat shall show the following:
(1) Identification.
A. Name of subdivision (shall be same as used on preliminary plan), township, tract and original lot, or section number.
B. Vicinity map at 1" = 1,000' or 1" = 2,000' scale with north arrow.
C. Date, north arrow, and scale (separate from vicinity map, north arrow, and scale).
(2) Control points. All dimensions, angles, and bearings are to be referred to control points, nearest established streetline, original lot line, or other established point. Control points must be either iron pin, pipe, monument, or stone, either found or set.
(3) Lines, boundaries and acreage. Center lines and right-of-way lines of streets, easements, and other right-of-way, natural and artificial water courses, streams, shorelines, corporation lines, lot lines, and property lines of all lots and parcels with distances, radii, arcs, chords, and tangents of all curves (nearest one-hundredth of a foot), bearings, or deflection angles (nearest second), and acreage of each lot (nearest thousandth) shall be shown.
(4) Streets. Name (shall not duplicate another in the Local Postal Zone zip code area) and right-of-way width of each street within proposed subdivision and those adjoining shall be shown.
(5) Building setback lines shall be accurately shown with dimensions. Minimum building setback shall be indicated.
(6) Lot and block identification. Lots shall be numbered in consecutive order and when the subdivision is submitted in sections or phases, lots shall be numbered consecutively as each section or phase is submitted.
(7) Every plat shall be superimposed on a survey of the lands of the dedicators from which such plat is drawn and shall contain an accurate background drawings of any metes-and-bounds descriptions of the lands of the dedicators from which such plat is drawn.
(8) Total site data shall be shown, including: acreage in roads, acreage in lots; total acreage; acreage of each lot and breakdown thereof should the subdivision fall in more than one lot; acreage in parks and other public uses; number of residential lots; and typical lot size.
(9) Land for public use. Boundaries shall show, and the use shall be identified of all parcels which are to be dedicated or reserved for public use or easements.
(10) Monuments. Location and description of monuments found, set, or to be set shall be shown.
(11) Iron pin not less than three-fourths (3/4) inch in diameter and not less than thirty (30) inches long shall be set as follows:
A. At all lot corners.
B. At center line intersection of all roads.
C. At the P.C. and the P.T. of all curves on the center line of the road and on all lot lines.
D. At all angle points.
Note: The Village Engineer will not accept the plat for recording a road for public use (see Ohio Revised Code, Section 711.09.1) unless pipes or pins have been set at the points indicated above or bond is posted to insure their placement as required.
(12) Names of recorded owners of adjoining unplatted land and reference to subdivision plats of adjoining platted land by name, volume, and page of the County Recorder's maps.
(13) Signature and seal of a registered surveyor to the effect that the plat represents a survey made by him which balances and closes, and that the monuments shown thereon exist, and that all dimensional and geodetic details are correct.
(14) Notarized certification by the owner or owners of the subdivision and the offer of the dedication of streets and other public areas and easements and that there are no unpaid taxes or special assessments against the land contained in the plat.
(15) Notation for:
A. Zoning approval by the Zoning Inspector.
B. Approval of plat by Secretary of the Perry Village Planning Commission.
C. Approved and accepted for record by the Perry Village Council.
D. Approved for record by Village Engineer.
E. Proper notations for transfer and recording by the County Auditor and the County Recorder.
F. Offer of dedication of streets, parks and easements by the owners of record.
G. Approval of the County Health Department that the subsoil conditions are suitable for permanent use of individual sewage disposal facilities where public sewers are not required.
H. Covenants, if any, and approvals for record by Village Solicitor.
