The application shall contain the following information:
A. Small business owner name, contact information, and name of business;
B. Property owner name and contact information;
C. Written description of the proposed business;
D. A drawing to scale of the property. The drawing should include primary business location, parking and access, and the location of the proposed small business on the site;
E. Signatures of the property owner(s) and small business owner(s). (Ord. 401, 5-14-2013)
