The following regulations shall govern sick leave for all employees:
(a) Sick leave is defined as leave with pay granted when an eligible employee is necessarily absent from duty for any of the following reasons:
(1) Illness or injury which makes him unable to perform his duties or a hazard to others in the performance of his duties;
(2) Exposure to contagious disease;
(3) Death or serious illness of a member of the employee’s immediate family (grandparents, spouse, children, parents, brother or sister);
(4) Time off to visit a doctor or dentist for medical or dental care.
(b) Sick leave shall be earned at the rate of four and six-tenths hours for each 80 hours of completed service including paid holidays and paid vacation. Unused sick leave shall be cumulative up to 120 work days, unless more than 120 work days are approved by the Mayor. Any employee who accumulates during a calendar year additional sick leave of more than 120 days shall have the excess paid for at the rate of one day’s pay for each two day’s earned so that at the end of the calendar year the employee’s accumulated total shall not exceed 120 days. Payment for such accumulation shall be not later than January 31st of the following year.
(c) Accumulated sick leave shall, except as provided below, be forfeited upon termination of employment, whether by permanent layoff, discharge for cause, voluntarily quit or retirement. If an employee retires, either regular or disability, or dies, in any case with at least 20 years of service with Pepper Pike, he shall be paid at his retirement for accumulated sick leave at the rate of one day’s pay for every two days accumulated. If an employee is rehired, previously accumulated sick leave forfeited on the prior termination is not reinstated. In the event of the death of a full-time employee, all of the remaining benefits hereunder shall be paid in accordance with prevailing state law, first directly to his surviving spouse, or if there is none, then to his estate as a lump sum, single payment.
(d) An employee must advise his superior at least one hour before he would normally report for work to receive sick leave.
(e) If an employee is on sick leave for more than three days in succession, he must provide a doctor’s certificate to his department head.

(f) An employee may donate sick leave to another employee upon the same terms and conditions for the donation of sick time by State employees under the provisions of Ohio Revised Code Section 124.391 and Administrative Code Section 123:1-46- 05 “Leave Donation Program”.
