250.055 MEDICAL INSURANCE.
   (a)   The City shall provide to all regular employees medical insurance to such extent and pursuant to such policy as shall be approved and provided by Council. The policy shall offer the option of a Health Savings Account as shall be approved and provided by Council.
   (b)   Employees, excluding exempt Director Level positions and any employees whose medical insurance contributions are either covered by a collective bargaining agreement or who are members of a collective bargaining unit for whom an agreement is the subject of pending negotiation, mediation, conciliation or arbitration, who elect the single coverage under the policy shall contribute $55.00 per month for such coverage and employees who elect family coverage under the policy shall contribute $130.00 per month for such coverage. The contribution shall be deducted from the employee’s compensation in two equal installments from the first and second pay periods of each month. Exempt Director Level positions shall contribute fifteen percent (15%) of the current premium in effect at the time of the contribution, regardless of whether the individuals in such positions elect the Health Savings Account option.
   The amount of employee co-pays to the extent permitted by applicable law are as follows:

   In addition to the above co-pays, there shall be an annual deductible of five hundred dollars ($500.00) for single coverage under the policy and one thousand dollars ($1,000) for family coverage under the policy, to the extent permitted by applicable law.
   (b)   The City shall provide to all regular employees the opportunity to enroll in a vision care insurance plan to such extent and pursuant to such plan as shall be approved and provided by Council. The employee shall pay 100% of the premium for the vision care insurance plan.
   (c)   The City shall provide to all regular employees the opportunity to enroll in a dental care insurance plan to such extent and pursuant to such plan as shall be approved and provided by Council. The employee shall pay 50% of the premium for the dental care insurance plan.
   (d)   Employees shall receive the benefits described in subsections (a), (b), and (c) in addition to their regular compensation.
   (e)   An employee whose spouse has available health care coverage at the spouse's place of employment but elects coverage of the spouse by the City's medical insurance plan shall be surcharged an additional two hundred dollars ($200.00) per month.
   (f)   An employee tobacco-free policy will be established and shall include a tobacco-free incentive program as part of a wellness program under which an employee free from usage of tobacco products will be incentivized upon terms and conditions set forth in the tobacco-free policy in accordance with the applicable law.
