315.03 CONTENTS OF PERMIT.
   The permit application shall clearly state:
   (a)   The name, address, telephone number, and facsimile transmission number, if applicable, of the person requesting the permit.
   (b)   The name, address, telephone number and facsimile transmission number, if any, of the organization for which the parade or public assemblage is to be conducted.
   (c)   A brief description of the activities planned for the event.
   (d)   The date requested for the parade or public assemblage.
   (e)   For a parade, a map showing the route to be traveled, including the commencement and termination points.
   (f)   The hours of assembly, commencement and termination for the parade or public assemblage.
   (g)   The approximate number of people, vehicles, and animals, if applicable, that will comprise the parade or public assemblage and a description of any vehicles or animals to be included.
   (h)   The approximate number of spectators expected.
   (i)   A list of any public facilities or equipment to be used.
   (j)   Any other relevant information not listed above.
