(A) Application for said registration certificate shall be made to the Village Clerk and shall contain the name and address of the applicant, the permanent address of the applicant, the name and address of the person, if any, for whose purpose the business will be concluded, a summary of the goods to be sold or the services rendered, a description of any vehicle proposed to be used in the business (including the license number) and the date or dates the business will be conducted within the municipality.
(B) Applicants shall provide proof of a current and effective commercial liability insurance, proof of a state sales tax permit, if applicable, and proof of a food establishment inspection permit, if applicable.
