§ 152.033 APPLICATION FOR A PERMIT OR APPROVAL.
   (A)   To obtain a floodplain development permit or approval the applicant shall first file an application in writing on a form furnished by the city.
   (B)   Application stage. The information provided shall:
      (1)   Identify and describe the development to be covered by the permit or approval.
      (2)   State the elevation in relation to the datum on the FIRM of the proposed lowest floor (including basement) of all structures.
      (3)   State the elevation in relation to the datum on the FIRM to which any nonresidential structure will be flood-proofed.
      (4)   Describe the land on which the proposed development is to be conducted by legal description, street address, or similar description that will readily identify and definitively locate the site.
      (5)   Provide a description of the extent to which any watercourse will be altered or relocated as a result of the proposed development.
      (6)   Indicate the use and occupancy for which the proposed development is intended.
      (7)   Be accompanied by a site plan or construction documents as specified in §§ 152.040 through 152.043 of this chapter.
      (8)   State the valuation of the proposed work.
      (9)   Be signed by the applicant or the applicant's authorized agent.
      (10)   Provide a certificate from a registered professional engineer or architect that the nonresidential flood-proofed structure will meet the flood-proofing criteria of the Florida Building Code.
      (11)   Give such other data and information as required by the Floodplain Administrator.
