(A) Permission to solicit donations of accrued leave from departmental personnel and/or city-wide personnel shall be submitted in writing to the department director of the absent employee. The department director shall consult with the Personnel Director, or designee, in reviewing such requests. Consideration shall be given to the designated employee’s previous leave history, as well as the nature of the illness or injury.
(B) Such written requests shall include employee’s name, reasons for requesting such donations of accrued leave and approximate duration of absence, if known.
(C) Upon approval of such request, the requesting department’s director shall obtain a supply of the donation form, prepared by the Personnel Department for application for donation of accrued sick/vacation leave, and shall distribute these forms to employees willing to donate accrued leave time.
(D) As forms are completed by the donors, the department director shall forward same to the Personnel Department who shall time date and stamp each form in the order it is received and deduct donated time from the appropriate balance. Donated time will be credited to the absent employee in the order in which the forms are received.
(E) Donated time will be converted to a dollar value based on the current rate of pay of each donor for hours donated and on the rate of pay of each donee for hours utilized. The rate of pay used for each donor will be that in effect at the time the application is signed.
(F) Time donated for this purpose will not be considered during the donor’s performance rating period, nor will it affect a donor’s right to convert accrued sick leave to cash payment, as established by the city’s Career System Personnel Rules and Regulations.
(G) The Personnel Director, or designee, shall notify the appropriate department director when donated time is nearly exhausted and that department shall have the responsibility of requesting additional donated time, if desired.
(H) The department director shall immediately notify the Personnel Director, or designee, by phone of the absent employee’s return to work or of any major change in the employee’s physical condition or employment status. In the event of excess donations received, but not used, the Personnel Department shall notify the department director of the donation forms voided and the time returned to the donating employees. Donated time returned to a donor shall be reflected in leave balances on the donor’s pay stub.
