§ 151.120 PERMIT REQUIRED.
   (A)   No person, firm or corporation shall hereafter construct, reconstruct, establish or maintain:
      (1)   Any public sidewalk or curb;
      (2)   Any driveway over, across or upon any portion of the public sidewalk or public parkway, without first having obtained a written permit to do so from the Building Administrator. No such permit shall be issued for construction, reconstruction, establishment or maintenance of any such public sidewalk, curb and/or driveway except in accordance with the provisions herein contained.
   (B)   Application for a permit must be made in writing upon forms furnished by the city. Said application shall contain the name and address of the person, firm or corporation making the application, the name of the contractor or person who is to construct said public sidewalk, curb and/or driveway and the proposed location and dimensions of such public sidewalk, curb and/or driveway. Complete plans and specifications shall be submitted to the Building Administrator and in accordance with this chapter; and shall be inspected upon completion by the Building Administrator.
