(A) The City Clerk shall issue, without charge, upon application of any disabled person or disabled veteran, a special disabled motorist decal or device furnished by the Illinois Secretary of State, pursuant to the provisions of the Illinois Vehicle Code (ILCS Ch. 625, Act 5, § 11-1301.2), for the purpose of allowing the decal or device to be used and displayed on a vehicle being used to transport the disabled person or disabled veteran, but not owned or displaying a special disabled or disabled veteran's license plate, so as to identify the vehicle and entitle it to all the parking privileges afforded to a vehicle possessing such special disabled or disabled veteran license plates as herein provided. The decal or device shall be issued only to disabled persons or disabled veterans who are residents of the city and the same shall be displayed along the right border of the front windshield of a vehicle and shall allow the vehicle to utilize special, disabled parking privileges when used to transport a disabled person or disabled veteran. The decal or device shall be valid for no more than 90 days subject to renewal for like periods based upon continued disability, and further provided that the decal or device clearly sets forth the date that the card or decal expires.
(B) All applicants must furnish a certified form designating the disability. All applicants must be residents of the city at the time of application and must furnish proof of the same. No disabled motorist vehicle card shall be transferable. The use of the card by any person other than the disabled person to whom it is issued except as provided herein shall be deemed a violation of this section and shall render the card void. Any abuse of the card or unauthorized use shall render the card subject to revocation. No disabled motorist card shall be issued until an application is completed and received by the City Clerk. Providing false information in the application filed with the City Clerk shall be deemed a violation of this section.
