1066.08 INFORMATION RETURN REQUIRED.
   Simultaneously with the remittance described in Section 1066.07, each telecommunications carrier shall make a return to the Cook County Emergency Telephone System Board for the period to which the remittance applies, stating the following:
   (a)   The name of the telecommunications carrier;
   (b)   The telecommunications carrier's principal place of business;
   (c)   The number of network connections to which the surcharge applies for the Village;
   (d)   The amount of the surcharge due; and
   (e)   Such other reasonable and relevant information as Council may require.
