No person shall make any connection to the sewer mains of the sewerage system of the Village, except upon written application to the Commissioner of Public Works and Streets or his or her designated representative and the issuance of a permit by such Commissioner for such connection. Each application shall state the name of the applicant, the permit desired, the location to be used and the fee to be paid. Each application shall contain such additional information as may be needed for the proper guidance of Village officials in the issuance of the permit.
