864.04 LICENSE APPLICATION.
   (a)   An application for a license to distribute unsolicited material or to engage in soliciting shall be made upon a form provided by and filed with the Village Clerk. The applicant shall truthfully state in full the following information on the application:
      (1)   The name of the applicant, his or her home address and the length of residence at such address, his or her business address if other than the residence address and his or her Social Security number;
      (2)   The home address of the applicant during the past three years if other than his or her present address;
      (3)   The age of the applicant, his or her marital status and, if married, the spouse's name;
      (4)   A physical description of the applicant;
      (5)   The name and address of the person by whom the applicant is employed or whom the applicant represents and the length of time of such employment or representation;
      (6)   The name and address of the applicant's employer during the past three years if other than the present employer;
      (7)   A description, sufficient for identification, of the subject matter of the distribution or solicitation which the applicant will engage in;
      (8)   The period of time for which the license application is made;
      (9)   The date, or approximate date, of the applicant's latest previous application for a license under this chapter, if any;
      (10)   Whether or not a license issued to the applicant under this chapter has ever been revoked;
      (11)   Whether or not the applicant has ever been convicted of a violation of any of the provisions of this chapter or the ordinances of any other Illinois municipality regulating the distribution of unsolicited material, soliciting or canvassing;
      (12)   Whether or not the applicant has ever been convicted of the commission of a felony under the law of the State or any other state or Federal law; and
      (13)   Such additional information as the Chief of Police may deem necessary to process the application.
   All statements made by the applicant upon the application or in connection therewith shall be under oath.
   (b)   The Clerk shall cause to be kept in his or her office an accurate record of every application received and acted upon, together with all other information and data pertaining thereto and all licenses issued under this chapter, and of the denial of such applications. Applications for licenses shall be numbered in consecutive order as filed, and every license issued, and any renewal thereof, shall be identified with the duplicate number of the application upon which it was issued.
   (c)   No license shall be issued to a person who has been convicted of the commission of a felony under the law of the State or any other state or Federal law within five years of the date of the application; to a person who has been convicted of a violation of any of the provisions of this chapter or the ordinances of any other Illinois municipality regulating distribution of unsolicited material, soliciting or canvassing, or to a person whose license has been previously revoked as provided in this section.
