Application for a dog license shall be made with the Department of Accounts and Finances. At the time of such application, the owner shall provide the following:
(a) The name and address of the owner of the dog;
(b) The name, age, sex, breed and description of the dog to be licensed; and
(c) A certificate of inoculation for rabies from the County Health Department bearing a date within one year prior to the date of application.
