(A) Certification by a registered land surveyor to the effect that the plat represents a survey made by him, that the monuments shown thereon actually exist as located, and that all dimensional and other details, such as curve data, are correct.
(B) Notarized certification by the owner or owners of the land, or other persons having an interest therein (including mortgagees), of the adoption of the plat and the dedication of streets and other public areas.
(C) Certification by the signature of the proper officials of Cook County that there are no tax delinquencies.
(D) Certification by the signature of the city treasurer that there are no special assessment delinquencies or other liens.
(E) Space for the approval of the plan commission by date and the signature of the chairman of the commission.
(F) Space for the approval of the city council by date and the signature of the mayor; and space for the signature of the city clerk, and for the city seal.
