§ 119.03 PERMIT REQUIRED.
   (A)   Any person or entity that wants to conduct a pyrotechnic display shall obtain a permit therefor. Applications for a pyrotechnic display shall be made in writing at least 15 days in advance of the date of the display, unless agreed to by the city's issuing officer and the Fire Chief or his or her designee of the jurisdiction in which the display will occur. No permit shall be issued by the city until the Chief of the Fire Protection District in which the display shall occur, or his or her designee, has inspected the site and determined that the display can be performed in full compliance with the rules adopted by the State Fire Marshall and that the display shall not be hazardous to property or endanger any person.
   (B)   Permits shall be signed by the Chief of the Fire Protection District in which the display is to occur, or his or her designee, and shall identify the lead pyrotechnic operator. The lead pyrotechnic operator of a pyrotechnic display must have a copy of the permit in his or her possession. Permits are not transferable.
   (C)   The application for a pyrotechnics display permit must be accompanied by a certificate of insurance, from an insurance company licensed to do business in Illinois, for general liability coverage with limits of $1,000,000 for bodily injury and $200,000 for property damage. This certificate of insurance shall name the city and its officers, agents and employees, and Fire Protection District as additional insured. In addition, the certificate shall specifically state on its face that the general liability coverage includes coverage relative to pyrotechnic displays and that the policy shall remain in effect for at least 30 days after the display. Only general liability insurance policies which are issued on an occurrence basis shall be considered sufficient to satisfy the insurance requirements of this section.
   (D)   The application for a pyrotechnic display permit shall be accompanied by a cash bond, in the amount of $1,000, conditioned upon compliance with all applicable city and fire codes and ordinances.
   (E)   The permit fee is $500 for each day of the pyrotechnic display. City sponsored events are exempt from all/any permit fees.
