An application for a food-service establishment license shall be made in writing to the health officer, and the required license fee shall accompany the signed application. The application shall be on a form provided for that purpose, and shall include the name and residence addresses of the persons desiring the same. If the applicant is a partnership or firm, the application shall contain the names and residence addresses of each of its members; if a limited partnership, the name and residence address of each general partner thereof, and if a corporation, the application shall contain the names and residence addresses of its principal officers. In addition to these statements, there shall be set forth in the application the location and type of the proposed food-service establishment, the number of persons to be engaged on the premises of the establishment, and other information as the health officer, in conformance with the provisions of this chapter, may require. The application shall be signed by the applicant or applicants.
