§ 111.04 CHARITABLE ORGANIZATIONS.
   Every charitable organization soliciting for charitable purposes, as those terms are defined in ILCS Ch. 225, Act 460, § 1(a) and (f) shall not be required to furnish the information called for under § 111.03 (1), (2), (3), and (4), if they furnish the City Clerk the following information:
   (A)   Name and address of organization.
   (B)   Nature of charitable service.
   (C)   Name and address of the person or persons having the primary responsibility for supervision of the solicitation within the city.
   (D)   Names and addresses of all persons who will be soliciting.
   (E)   Dates and locations of intended solicitations.
   (F)   Evidence of full compliance with the provisions of "An act to regulate solicitation and collection of funds for charitable purposes, providing for violations thereof, and making appropriation therefore," approved July 26, 1963, as amended.
   (G)   Organization shall be liable for any injuries to any person or property during solicitation which is causally related to an act of ordinary negligence of the soliciting agent. Evidence of adequate liability insurance shall be required prior to the issuance of any certification of registration for the purposes described in this section.
   (H)   Such additional information as the City Clerk may deem necessary to process the application.
