§ 110.08 FORMS TO BE KEPT BY CITY CLERK;INFORMATION TO BE SET FORTH.
   Forms for all licenses and permits and applications therefor shall be prepared and kept on file by the city clerk. These forms shall state the fee paid thereon, duration of the term, and when issued.
