(A) Applications for all licenses and permits required by this chapter shall be made in writing to the city clerk in the absence of provision to the contrary. Each application shall state the name of the applicant, the permit or license desired, the location to be used, if any, the time covered, and the fee to be paid, and each application shall contain such additional information as may be needed for the proper guidance of the city officials in the issuing of the permit or license applied for.
(B) All retail business license applications, new or renewals, must be accompanied by that business' Illinois retail occupation tax number. All new applications will be issued a temporary license clearly stating that the license is conditional upon receiving that business' Illinois retail occupation tax number within 60 days. All renewals that do not include the number will be rejected.
