§ 91.21 LICENSE ISSUED BY CITY CLERK.

   The city clerk's office shall issue and renew all annual animal licenses and corresponding animal collar tags to the owner. The application shall include the following information:

   (A)   Date of application;

   (B)   Tag number;

   (C)   Owner's name, address and telephone number;

   (D)   Animal's name, breed, color, sex, and age;

   (E)   Rabies certification number and date of vaccination; and

   (F)   Veterinarian's name, address, and telephone number.
