(A) Fees for special permits to move vehicles, combinations of vehicles, and loads with overweight-axle loads shall be paid by the applicant to the department as follows.
(B) For each overweight single axle or tandem axle group, the flat rate fees herein scheduled for increments of 45 miles or fraction thereof including issuance fee predicated upon an 20,000 pound single axle equivalency.
