§ 50.04 APPLICATION INFORMATION.
   Each application for a water or sewer connection permit shall state the name of the applicant, the permit desired, the location where the permit is to be used, and the fee to be paid, and each application shall contain such additional information as may be needed for the proper guidance of the city officials in the issuance of a permit.
