§ 33.34  PURCHASE REQUEST RECORD.
   The purchaser shall maintain a purchase request record.
   (A)   The record shall reflect the purchase order number and the department making the request.
   (B)   The record shall reflect the vendors who were contacted, the prices quoted and the date of such quote.
   (C)   The record shall reflect which vendor was selected and the final purchase price.
   (D)   The record shall reflect, when only a single vendor was contacted and the requestor who made the determination of fairness of price and need for the item.
