§ 150.20  PERMIT APPLICATIONS.
   Applications for excavation or fill permits shall be on forms established for that purpose, which forms shall be available at the City Clerk's office at city hall, and which shall require of each applicant at least the following information:
   (A)   The name and address of the applicant.
   (B)   A detailed map or drawing showing the excavation or fill site and all lots, tracts or parcels of surrounding or adjoining property owners, detailing all buildings located thereon.
   (C)   A statement of the nature and extent of all water flow (stream, course, drainage) to be in any way affected.
   (D)   The purpose or reason for the excavation or fill.
   (E)   The length, width, breadth and depth of all excavations or fills, and the distance of such excavations or fills from the lines of all abutting property owners.
   (F)   The name and address of the owner of the lot, tract or parcel on which the proposed excavation or fill is to be located, and the names of the owners of all property adjoining, abutting or touching the lot, tract or parcel.
