905.03 APPLICATION.
   (a)   No excavation permit shall be issued unless a written application for the issuance of an excavation permit is submitted to the Recorder. The written application shall give the location and dimension of the proposed opening, the purpose for which the opening is made, the kind of pavement or surface to be opened and the approximate time when the cut will be made.
   (b)   Application for such a permit shall be made in writing to the Recorder on forms supplied by the Municipality. Such application shall contain at least the following:
      (1)   The name and address of:
         A.   The applicant;
         B.   The owner of the land on which the work will be performed; and,
         C.   The contractor performing the work.
      (2)   An identification and description of the work to be covered by the permit for which application is made.
      (3)   A description of the land on which the proposed work is to be done, by lot, block, tract, and house and street address, or similar description which will readily identify and definitely locate the proposed work.
      (4)   An estimate of the value of the proposed work.
      (5)   Such other information as reasonably may be required by the Building Permit Officer.
      (6)   The proposed starting and completion dates to accomplish the work.
