751.02 APPLICATIONS FOR AND ISSUANCE OF LICENSES.
   (a)    Any person who desires a license provided for in this article shall make an application to the Recorder on forms to be prescribed and furnished by the Recorder. At the time of application, the requestee of the license shall tender the license tax for each license certificate requested.
   (b)    Applicants who are holders of business franchise certificates or other forms of State licenses authorizing them to do business within the Municipality shall display such State certificates or licenses to the Recorder at the time of making application for the Municipal license. Upon collection in full of the proper taxes and fees, imposed by this article and the determination to his satisfaction that all the conditions precedent to the granting of any license have been met by the applicant, the Recorder shall issue the certificate of license. Failure, on the part of the applicant, to fulfill the precedent conditions shall constitute grounds for denial.
   (c)    For each license issued, but not for certificates of license granting free permits or free licenses, the Recorder shall charge a certificate fee of fifty cents (50¢). Any licensee who states under oath or by affirmation that he has lost his license certificate or that such certificate has been destroyed or mutilated shall be issued a duplicate of such certificate (plainly marked as such) upon payment of a certificate fee of one dollar ($1.00).
   (d)    If any license requires the certification of some other official before issuance thereof, the Recorder shall provide the applicant with a form on which to obtain such certification and such form, when returned with the required certification shall be filed with the Recorder.
