(a) Any person eligible for office may become a candidate and have his name placed upon the ballot, upon his own motion, by filing with the City Recorder a declaration of his candidacy and a certificate of his eligibility, which certificate shall be in form or effect as follows:
I hereby certify that I am a candidate for the office of and desire my name printed on the official ballot to be voted for at the city election to be held on the day of , 20 ; that I am a qualified voter of the City of Paden City, Tyler and Wetzel Counties, State of West Virginia; that I reside in ward No. in said City; and that my residence number is in said City; that I am eligible to hold said office; and that I am a candidate for said office in good faith.

(b) Such certificate shall be filed within sixty days before the day of the election, and must be received before midnight, eastern standard time, of the day or, if mailed, shall be postmarked before that hour.
