The voter registration forms for County-State elections and for Municipal elections shall be identical, except for color. The forms shall be prescribed by the Secretary of State and may be by him revised and supplemented from time to time so as to provide thereon for a continued record of voter registration and voter election participation. These forms shall be kept with County Clerk and Municipal registrations are to be made when a citizen registers for the County-State elections.
