§ 98.027 PERMITS.
   (A)   On state highways, the town street opening permit will be required before the state permit will be issued. This subchapter will not supercede any regulations as set forth by the State Department of Transporatlon in regards to excavation in state highways.
   (B)   Permits will not be issued until the applicant has:
      (1)   Paid to the town a permit fee of $5 for each opening, excavation and the like;
      (2)   Filed with the town a surety bond of $10,000 from a regular indemnity or surety company authorized to do business in the state, except those public utility companies which, under the Statutes of Connecticut, are exempt;
      (3)   The applicant shall file his or her business address with the Director of Public Works and shall notify him or her promptly of any changes therein. Any orders of notices the Director’s office may have to give to the applicant, mailed to the address so filed, shall be considered as due notice delivered to him or her personally;
      (4)   Shall file and maintain name or names of persons, telephone numbers and addresses who may be contacted in case of emergency after regular working hours; and
      (5)   Called the State Underground Utility Protection Plan (C.U.U.P.P.) (1-800-922-4455) prior to starting his or her excavation and shall give the assigned C.U.U.P.P. number to the Town Department of Public Works.
