Digital copy in Auto CAD format submitted on a CD. Shall utilitize the NAD-83 Ohio South State Plane Corrdinate System for horizontal control and the NAVD-1988 for vertical control. All subdivision boundary corners shall also be tied to the NAD-83 Ohio South State Plane Coordinate System and labeled with a northing and easting, as well as centerline intersections, temporary turn-around center points and easement centerline that does not follow a lot line. AutoCAD layouts shall be provided allowing for the reproduction of a complete set of plans as needed.

The name, mailing address, and telephone number of the applicant and owner.

Metes and bounds description of the property (Legal Description).

Proof or receipt of accepted bond filed with City Clerk.

Record Plat shall show the following: (Cover Sheet showing whole subdivision layout with specific Phase delineated for recording. Second sheet should show specifics required.)
A.    Name of Subdivision and Vol./Bk. _____ Pg. _____ for plat.
B.    The seal, registry number and signature of the registered surveyor and professional engineer who prepared the plat.
C.    North arrow, scale and date.
D.    Boundary lines, size and tract of whole site and each individual lot in square feet and acres along with the linear measurement thereof.
E.    The names and boundaries of all adjoining subdivisions and the names of recorded owners of adjoining parcels including parcel and lot numbers, including Deed Bk. & Pg. numbers within200 feet of the site.
F.    Locations, dimensions, widths, and names of existing street, alleys, lots, railroads, right-of-ways, easements, and parks.
G.    Location, dimensions, widths and names of proposed streets.
H.    The radii, arcs. lengths, points of tangency and central angles for all curvilinear streets and the radii and lengths for rounded corners.
I.    All new utility easements shall be shown. Water lines, gas lines, electric lines, telephone lines, sanitary sewer and detention/ rentention.
J.    The location of all survey monuments and benchmarks together with their descriptions. There shall be a minimum of four permanent markers in each subdivision section located at or near the outer corner.
K.    The zoning district in which the site is located and statement certifying that the subdivision satisfies all applicable requirements of the zoning district governing the use of the land at the time of approval.
L.    Building setback lines and dimensions.
M.    Lot numbers secured from the County Auditor.
N.    Restrictions and covenants included in the deeds to the lots of the subdivisoin (HOA Documents). Reference to such instrument shall be made on the plat as well as a Bk. _____ Pg. _____ reference to fill-in.
O.    Reference City Ordinance No.'s from Preliminary & Final Plat approvals and date of Approved Construction Drawings.
P.    Acknowledgement by the owner or owners fo the plat restrictions inclulding dedication of public streets, parks, alleys and granting required easements. (Requires Notorized Signatures)
Q.    Approval and acceptance clause for signatures of City Representatives.
